Congested wire space in breaker panel

I have relatively new 200 Amp service to my home, installed by licensed electricians. The breaker box is a Siemens, 40 slot panel. I have been adding new 15 Amp and 20 Amp circuits. The panel space (volume) available to connecting ground wires and to a lesser degree, neutral wires is becoming very congested. Questions:
1.The panel label appears to indicate that installation of additional grounding bars in possible. Can this be accomplished without taking the panel off a cinder block basement wall... or
2. Could/should I insert more than one ground wire (each from different circuits) under a single lug on a grounding bar.
3. Same questions for neutral bars.
